Applicator Train-the-Trainer Program Prerequisites

Prior to going through the Applicator Train-the-Trainer program, you must provide information showing that you meet the minimum requirements as set forth in this form. If SSPC is unable to verify the accuracy of any information reported on this form, it may result in rejection of your application.

The following items must be submitted to SSPC two weeks before the scheduled course.

• Application Form
  (Relevant work experience hours MUST be included)
• Picture for your wallet card
  (.jpg file of no more than 1 inch by 1 inch)
